- A person who wishes to drink wine during his meal, must recite the separate blessing of borei pri ha-gafen (“Who creates the fruit of the vine”) because wine has special status and it is not included in the blessing of Hamotzi (“Who brings forth bread from the earth”) which he recited over the bread. Even if the person specifically had the wine in mind when reciting the Hamotzi, it is not exempted(Shulchan Aruch, OC 172 with Mishnah Berurah).
2. On Shabbos and Yom Tov, when Kiddush is recited over wine before the meal, that blessing over the wine exempts the wine the person will drink during the course of the meal. But in places where it is not customary to drink wine during the meal, even on Shabbos, then the borei pri ha-gafen recited before the meal does not exempt wine the person decides to drink during the meal — unless the person specifically had this in mind from the beginning, at the time of Kiddush.
