Laws of Lashon Hara / 4: 5-6 (43)

5. Two people that witness another committing a sin, if he is known to hate rebuke, and he will easily commit the sin again, they are permitted to tell over his act only to someone of authority or to one of his relatives, so that they may punish him and prevent him from sinning once again. If a single person was witness to the act, he is forbidden to pass it one as he is considered non trustworthy. If however, he knows that his rabbi will rely on him to the same extent that he would rely on two witnesses, he is permitted to tell him, however his rabbi is forbidden to pass it on to others.

6. It seems that even when one’s rabbi is not good at keeping a secret, if there is benefit to what he tells him, as it will prevent the subject from committing the sin again, he is permitted to tell him.
